Please complete our new patient form in advance, or arrive about 10 minutes early for your first appointment so you have enough time to fill it out.

Initial Examination
Your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any required X-rays.
Treatment Plan
Based on the findings of your history and examination, your dentist will develop a long-term treatment plan to help improve and maintain your oral health. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

- How do I cancel or reschedule an appointment?
If you need to cancel or reschedule an appointment for any reason, call us as soon as possible. Please let us know about any change of plans as soon as you can, so we can offer the space in our schedule to another patient.
